Notes: This is a PC copy of the numbered issue, limited to 350 numbered books and a small group of PC (publisher's) copies. Paul Suntup gave this PC copy to Michael Russem in acknowledgement of his work as the book's designer.

According to the Suntup website, the numbered edition has "a quarter leather binding with Corkskin paper sides handmade in Portugal. The cover features a letterpress printed label and endsheets are Hahnemühle Bugra. The edition is printed letterpress on Mohawk Superfine and is housed in an embossed paper covered slipcase with an acrylic topcoat. The edition is signed by Bradford Morrow [introduction] and David Álvarez [illustrations]." This copy is also signed by Russem on the colophon.

Michael Russem designed and printed innovative fine press books for his own Kat Ran Press for many years; he now runs a design bookstore and gallery near Cambridge, Massachusetts, and he also designs books for many publishers. Among the books he designed for Suntup Press include Slaughterhouse-Five, Silence of the Lambs, Blood Meridian, and The Handmaid's Tale.

With six full-page color illustrations by Alvarez.
